چکیده

Materials and methods During a 7 year period from October 2000 to April 2007, 84 symptomatic carotid stenosis were treated at Department of general surgery of AUO “Federico II” of Naples 48 with CAS and 36 with CEA. We divided our patients in two group not randomly but according to valuation anesthesia. The age range in this group was 80 to 89 years (average age of 82.7 years). The indication for treatment were transient ischemic attack in (43.1%). Cerebrovascular accident in (23%), amaurosis fugax in (9.3%), vascular tinnitus in (2.6%) Associated risk factors included systemic arterial hypertension, diabetes mellitus, coronary artery disease and significant smoking history. All procedures were performed under local anesthesia. Concomitantly or during the same hospitalization two patients underwent adjunctive procedures (coronary artery bypass, lung resection, colon resection).

Temporal Changes in Periprocedural Events in the Carotid Revascularization Endarterectomy Versus Stenting Trial.

BACKGROUND AND PURPOSE Post-hoc, we hypothesized that over the recruitment period of the Carotid Revascularization Endarterectomy Versus Stenting Trial (CREST), increasing experience and improved patient selection with carotid stenting, and to a lesser extent, carotid endarterectomy would contribute to lower periprocedural event rates. METHODS Three study periods with approximately the same n...

Carotid artery stenting in octogenarians: periprocedural stroke risk predictor analysis from the multicenter Carotid ACCULINK/ACCUNET Post Approval Trial to Uncover Rare Events (CAPTURE 2) clinical trial.

BACKGROUND AND PURPOSE Carotid ACCULINK/ACCUNET Post Approval Trial to Uncover Rare Events (CAPTURE 2) is an ongoing prospective, nonrandomized, multicenter clinical trial enrolling patients undergoing carotid artery stenting. The aim of this analysis is to identify risk predictors for periprocedural stroke in patients > or = 80 years old. METHODS Symptomatic patients with > or = 50% stenosis...

ANGIOPLASTY AND STENTING OF CAROTID ARTERY STENOSIS WITH EMBOLIC PROTECTION DEVICES

Background: Carotid artery stenting (CAS) has recently been recommended as an alternative to carotid endarterectomy (CEA) by some clinicians. Objective: This study was designed to evaluate the success rate and in-hospital and 30-day adverse events in our first experiences in Iran for CAS with protection devices, to document our results and guide further use of CAS. Methods: From December 2...

Carotid artery disease in octogenarians: endarterectomy or stenting?

AIM Carotid endarterectomy (CEA) is the gold standard for the treatment of carotid stenosis, but carotid angioplasty and stenting (CAS) has emerged as a potential less invasive therapeutic alternative to patients who would otherwise be considered as high risk for open surgery. The aim of this review was to pool the most current studies of the growing body of literature in which outcomes of CEA ...

Octogenarians and nonagenarians with severe symptomatic and asymptomatic carotid disease: does older age indicate "high risk" for carotid endarterectomy?

Background Patients older than 80 years old were excluded from participation in the large controlled randomized trials that demonstrated the efficacy of carotid endarterectomy (CEA) in preventing stroke in selected symptomatic and asymptomatic patients. Because of limited longevity and perceived increased perioperative risk from CEA, alternative treatment options, such as carotid angioplasty an...

Carotid Revascularization Endarterectomy Versus Stenting Trial (CREST): current and future implications for carotid artery stenting

Current reimbursement guidelines from the Centers for Medicare & Medicaid Services, prior to the publication of the Carotid Revascularization Endarterectomy Versus Stenting Trial (CREST) results, reserve carotid artery stenting for stenosis in patients who are symptomatic, with severe stenosis, and considered high risk for carotid endarterectomy (CEA). CREST is a prospective, multicenter, rando...
